1. Intervertebral Disc Prolapse (Herniated Disc)

Cause: Incorrect posture causing intervertebral disc pushes out, pressing on nearby nerves. Herniated disc may cause sharp pain at buttock, radiating pain from buttock down to legs including numbness or tingling sensation, especially when moving.
Solution: Rest, physiotherapy and pain medications are the first line of treatment. Physiotherapy can help strengthen the muscles to stabilize the spine and reduce pressure on the nerves. If symptoms don’t improve, surgery may be needed.
2. Piriformis Syndrome

Cause: Piriformis muscle in the buttocks spasms and compress on the sciatic nerve, causing pain that radiates down the leg. Pain usually worse after prolong sitting.
Solution: Stretching and strengthening exercises for the hips and buttocks can help relieve the spasms. Physiotherapy can improve flexibility and reduce discomfort.
3. Gluteus Maximus Strain

Cause: The gluteus maximus is the largest muscle in the buttocks. Overstretching or overuse can lead to a strain, causing pain, swelling, and difficulty walking or sitting.
Solution: Rest, ice, and gentle compression help reduce pain and swelling. Once the pain subsides, specific exercises can help rebuild strength and mobility in the muscle.
4. Gluteus Medius Syndrome

Cause: The gluteus medius muscle helps in hip stabilization. Injury of gluteus medius muscle can cause pain in the outer hip and buttock, along with weakness and changes in walking pattern.
Solution: Physiotherapy is key to strengthening the gluteus medius and improving hip stability. Targeted exercises can help restore muscle function and reduce pain.
5. Ligament Injury
Cause: Injuries to the ligaments around the pelvis can result from trauma or overuse. This can cause pain, swelling, and difficulty moving the hips.
Solution: The R.I.C.E. method (Rest, Ice, Compression, Elevation) can help manage pain and swelling. After the initial healing phase, physiotherapy can restore movement and strength. Severe cases may require further evaluation.
Simple stretching to relief buttock pain
1. Figure of 4 stretch
Place your leg into the position of “4”, put your hand on the ankle and knee without put pressure on it, slowly bend your body down with back straight, when started to feel a pulling sensation on a scale of 3/10 hold for 20 seconds for 3 times. Perform for both sides.


2. Gluteus medius stretch
Place your leg into the position as shown, use your opposite hand to pull your knee towards your opposite shoulder and slightly turn your body towards the knee, when started to feel sense of pulling over your buttock on a scale of 3/10 hold for 20 seconds for 3 times. Perform for both sides.
